I understand these risks and voluntarily choose to participate. Assumption of Responsibility I accept full responsibility for any injury or damage that I may sustain or cause to others while participating in this basketball league, whether during games, practices, or while on the premises of any facility used by the league. Release and Waiver In consideration of being allowed to participate, I hereby release, waive, and hold harmless [Alex Garner/Clearfork Hoops], its officers, agents, sponsors, facility owners, and other participants from any and all liability, claims, or causes of action arising out of or related to my participation, including negligence on the part of the released parties. Medical Authorization In the event of an injury, I authorize league officials or facility staff to secure medical treatment deemed necessary, and I agree to be responsible for any resulting medical expenses. Code of Conduct I agree to follow all league rules, facility regulations, and instructions provided by officials or organizers. I understand that failure to do so may result in suspension or removal from the league without refund. Photography & Media Release (Optional) I grant permission for photos or videos taken during league activities to be used for promotional purposes without compensation. Acknowledgment & Signature I have read and fully understand this waiver and release.
