Parental Consent I, [Parent/Guardian Name], as the parent or legal guardian of [Participant’s Name], give my full consent for my child to participate in all activities, training, games, travel, and events organized by MBK Basketball. I understand that participation in basketball and related activities involves risks of injury, including but not limited to, sprains, fractures, concussions, and other physical harm. I confirm that my child is in good health and has no medical conditions that would prevent safe participation. I agree to inform MBK Basketball of any relevant medical conditions, allergies, or special requirements. II. Assumption of Risk & Waiver of Liability I acknowledge and understand that basketball is a physically demanding sport that carries inherent risks. I hereby assume all risks associated with my child’s participation, including potential injury, accidents, or illness. I, on behalf of myself, my child, and our heirs, executors, and representatives, hereby release, waive, and discharge MBK Basketball, its directors, coaches, employees, volunteers, affiliates, sponsors, and facility providers from any and all liability, claims, demands, or causes of action related to injury, loss, or damages sustained by my child while participating in MBK Basketball activities, including but not limited to: • Injury caused by falls, contact with other players, equipment, or facilities. • Injuries occurring during travel to and from games or training sessions. • Medical conditions that arise before, during, or after participation. • Exposure to infectious diseases, including but not limited to COVID-19. III. Medical Treatment Authorization In the event of an emergency, I authorize MBK Basketball’s coaches, staff, or designated representatives to seek and obtain medical treatment for my child if I am unavailable. I understand that MBK Basketball is not responsible for medical expenses incurred as a result of injuries sustained during participation. I agree that I am responsible for any medical costs and that I will ensure my child has appropriate medical coverage during participation. IV. Photo & Media Release I give permission for MBK Basketball to take photographs, videos, or other media recordings of my child for promotional, social media, and marketing purposes. I understand that these images may be used on MBK Basketball’s website, social media pages, promotional materials, and other platforms. If I wish to revoke this permission, I will provide written notice to MBK Basketball. V. Code of Conduct & Compliance I acknowledge that my child must follow all MBK Basketball rules, codes of conduct, and safety guidelines. I understand that failure to comply with these standards may result in suspension or removal from the program without refund. VI.
