I hereby certify that I or my child have no physical defects that would prevent us from participating in the program. I hereby specifically agree that A Vision for You Help Center, its directors, officers, employees, agents, or volunteers are under no obligation to provide a physical examination or other evidence of my fitness or my child's fitness to participate in such program, the same being my sole responsibility. I here by agree to abide by all rules and regulations set by A Vision for You Help Center-Under the Lights Flag Football League. I agree to hold harmless organizers, supervisors, and participants for any injury and for any known or unknown illness that may occur while participation in the above league. REGISTRATION FEES ARE NON REFUNDABLE. League play may result in a serious injury and risks involve twisting ankle, being hit by football, jammed fingers, and more serious injuries not listed. Although I fully understand those risks, I desire to participate without regard to the consequences. I assume all risks and hazards incidental to such participation and do hereby waive, release, absolve, indemnify, and agree to hold harmless A Vision for You Help Center and any and all sponsors, or any other individuals, that take place in connection with this league. This waiver shall be binding on my heirs, legatees, administrators, and assigns.
