If you are Under 18, you must ask your parents to read the waiver and input their name and QID. The novel coronavirus 2019 (COVID-19), was declared a worldwide pandemic by the World Health Organization in March 2020. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact via water droplets. It is believed that an individual can be infected with COVID-19 without their knowledge and be asymptomatic. By writing your name into this agreement and showing up to participate in the league, I/I in my capacity as a parent or legal guardian hereby acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I/my child(ren) and I may be exposed to or infected by COVID-19 by attending the Touch Rugby League and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of myself and / or others, including, but not limited to, Qatar Rugby Federation employees, volunteers, participants and their families. I/ or I as parent or legal guardian, I agree to comply with all precautionary procedures communicated by the DRFC, including but not limited to, monitoring myself or my child(ren) for COVID-19 symptoms and not attend Touch Rugby or any other event should they be symptomatic or subject to isolation and/or quarantine requirements, complying with temperature check requirements when participating in training or any other event, and ensuring the QRF or the League Manager are informed of actual or potential risk of exposure to COVID-19. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child may experience or incur in connection with mine or my child’s attendance or participation in the Touch Rugby league. (“Claims”).
