(2025)Please read the following waiver carefully. By signing below, you are agreeing to the terms and conditions of this waiver. Assumption of Risk By participating in Valley Premier League in the Bronx, I acknowledge that soccer is a contact sport that involves physical activity, and I am aware that playing or participating in the League presents inherent risks of injury, including but not limited to sprains, fractures, concussions, and other bodily harm. I voluntarily accept and assume all such risks involved in my participation in the League, whether those risks are known or unknown. Release of Liability In consideration for being allowed to participate in the League, I, the undersigned, hereby waive, release, and hold harmless the League organizers, officers, coaches, volunteers, agents, sponsors, and fellow participants from any and all liability, claims, demands, causes of action, or suits of any kind, arising out of or in connection with my participation in the League, including but not limited to any injuries, damage to personal property, or loss of life that may occur during practices, games, or any other event associated with the League. Medical Consent In the event of an emergency, I give my consent for emergency medical treatment, including but not limited to medical attention, transportation, and hospitalization, should the need arise. I agree to bear all costs associated with such medical Health Certification I certify that I am in good physical health and capable of participating in the League. I affirm that I have no known medical conditions or disabilities that would prevent me from engaging in physical activity or that would pose a risk to my health while playing soccer. Code of Conduct and Behavior I agree to comply with all rules, regulations, and policies of the Valley Premier League. I understand that any inappropriate behavior, including unsportsmanlike conduct, violent actions, or disrespectful behavior toward fellow participants, staff, or referees, may result in my removal from the League without refund. Photo and Video Release I grant permission to The Valley Premier League and its affiliates to take photographs, videos, or other media of me during League events. I understand that these images or videos may be used for promotional purposes, including but not limited to social media, marketing materials, and advertisements, without compensation or further permission. Acknowledgment of Understanding By electronically signing this waiver, I acknowledge that I have read, understood, and voluntarily agreed to the terms outlined above.
