I understand that participation in football involves physical activity and inherent risks, including the risk of injury. In the event of an emergency and I cannot be reached, I authorize So Icey Boyz coaches, staff, or representatives to obtain emergency medical treatment for my child. I understand that I am financially responsible for any medical expenses incurred. I acknowledge and assume all risks associated with my child’s participation and hereby release, waive, and hold harmless So Icey Boyz, its coaches, staff, volunteers, affiliates, facility operators, and event organizers from any and all claims, demands, or liability for injury, illness, or loss arising from participation, except in cases of gross negligence. I grant permission for So Icey Boyz to photograph, video record, and use images or recordings of my child for promotional, marketing, or informational purposes, including social media and websites, without compensation. I agree that my child and I will follow all team rules, policies, and expectations, including standards of sportsmanship and conduct. I understand that failure to comply may result in removal from tryouts or the program without refund.
