I acknowledge that these risks may result in serious injury, disability, or even death. Release of Liability I hereby release, waive, and discharge G-Elite Basketball, its coaches, staff, volunteers, sponsors, and facility partners from any and all liability, claims, demands, or causes of action arising out of or related to any loss, injury, or damage sustained while participating in any G-Elite Basketball event, practice, or competition. Medical Authorization In the event of a medical emergency, I authorize G-Elite Basketball and its representatives to obtain medical treatment for my child. I understand that I am responsible for any medical costs incurred as a result of injury or illness during participation. Code of Conduct I agree that my child will follow all rules, instructions, and guidelines set forth by G-Elite Basketball staff. Any behavior deemed inappropriate, unsafe, or detrimental to the program may result in disciplinary action, including suspension or removal from participation.
