I knowingly and freely assume all such risks and full responsibility for my participation. I willingly agree to comply with all stated and customary terms, conditions, and rules for participation. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless OTS Elite Volleyball Academy, its officers, agents, coaches, staff, volunteers, and sponsors from any and all claims, demands, losses, damages, or liability arising from participation, whether caused by negligence or otherwise, to the fullest extent permitted by law. I understand that OTS Elite Volleyball Academy does not carry or maintain health, medical, or disability insurance for participants—each participant is expected to obtain their own coverage. Medical Authorization In case of injury or illness, I give my consent for OTS Elite Volleyball Academy and its representatives to obtain emergency medical treatment for the participant. I understand that I am responsible for any and all medical expenses incurred. Photo and Video Release I hereby grant OTS Elite Volleyball Academy, its representatives, and assigns permission to use photographs, videos, and likenesses of the participant for promotional, educational, or informational purposes in print, video, or digital media (including social media and the academy’s website), without compensation. I waive any right to inspect or approve the final product and understand these materials become the property of OTS Elite Volleyball Academy. Acknowledgment and Signature I have read and understood this waiver and release.
