I hereby authorize the coaching staff of Triple Threat Basketball Club/Danbury Hoops to act for me according to their best judgment in any emergency requiring medical attention. I hereby release, discharge and indemnify Triple Threat Basketball Club/Danbury Hoops, coaches, volunteers, and affiliated entities and their officers from and against any and all liability or causes of actions arising out of or in connection with my child's participation in the program. During the season, there may be occasions when pictures and videos are taken of children during games and special events. Triple Threat/Danbury Hoops understands that I may be sensitive to having my child(ren) in the local paper, on Danbury Hoops websites, or on Danbury Hoops social media pages.
