This participation form is signed with the understanding that organizers, volunteers, participants, and those involved with the Catalonia Soccer Camp: Winter 2020 will not be held responsible for any sickness or injury that my child, I, or anyone accompanying me may receive while in attendance or participating in any of the Catalonia Soccer Camp: Winter 2020 activities (including practices, games, meetings, and other events). If the parent/guardian cannot be contacted in case of injury or medical emergency, I give permission to the Catalonia Soccer Camp: Winter 2020 volunteers to render first aid or call 911 (at my expense). No insurance is provided by the Catalonia Soccer Camp: Winter 2020. Playing in Higher Age Group: If my child is younger than the listed age group for the team they are participating, I understand the risks and allow my child to play in the higher age category. Drop-off/Pick-up: Parents/Guardians are responsible for immediate pick-up of athletes from camp events. Parents must remain in vicinity of fields for duration of training and are responsible for child accountability. If any issues, parent may be asked to retrieve child. Photography: I will allow my child to be photographed at Catalonia Soccer Camp: Winter 2020 events. If not, I will notify the Catalonia Soccer Camp: Winter 2020 in writing. Uniforms: Uniforms aren't mailed, they must be picked up in-person. Details about pick-up will be provided. Uniforms and balls may not be provided, depending on sponsorship. Refund: The camp is free, so no refund or compensation will be offered if camp is canceled, interrupted, or changed in any way. Mandatory COVID-19 Notices: Parents or guardians may choose to either pick up their camper, or to let the camper remain and trust the camp to take appropriate safeguards, when informed by the camp operator that a child at camp has tested positive for COVID-19. There is an enhanced risk for anyone age 65 or older being in direct contact with athletes for 14 days after participating in a sport event or practice. Parents should monitor the health of their children and not send them to participate in sporting activities if they exhibit any symptom of COVID-19, nor should anyone exhibiting symptoms participate or attend. COVID-19 Health Guidelines: No parents or guardians allowed on fields during or between camp sessions, except to drop-off and pick-up campers. Parents who wish to spectate are encouraged to do so from their vehicles or along inner or outer fence line. Minimize in-person contact with others not in the individual’s household.
