{"data":{"waiver":{"id":"Wz14GgQ","type":"waiver","url":"https://playpass.com/go/Wz14GgQ","name":"FUTSALTX","status":"published","sign_method":"checkbox","allow_signature_reuse":true,"content":"FUTSALTX PARTICIPANT WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK & MEDICAL AUTHORIZATION In consideration for being permitted to participate in FUTSALTX tournaments, league games, training sessions, open play, watch parties, community events, and related activities, I acknowledge and agree to the following: 1. Participant Information This agreement applies to the participant identified during registration and is signed by the participant’s parent or legal guardian when the participant is under 18 years of age. 2. Acknowledgment and Assumption of Risk I understand that futsal, soccer, athletic training, tournaments, and related activities involve inherent risks, including but not limited to: Contact or collisions with other participants Falls, slips, trips, or loss of balance Contact with balls, goals, walls, flooring, equipment, or playing surfaces Sprains, strains, fractures, concussions, cuts, bruises, or other injuries Heat-related illness, dehydration, fatigue, or physical overexertion Acts or omissions of other participants, spectators, coaches, officials, volunteers, or staff Travel to, from, or between activities Serious injury, permanent disability, or death I understand that these risks cannot be completely eliminated, even when reasonable safety measures are taken. I knowingly and voluntarily accept and assume all risks associated with participation, whether known or unknown, foreseeable or unforeseeable. 3. Health and Fitness Certification I certify that the participant is physically able to participate in athletic activity. I agree to disclose any medical condition, allergy, injury, medication, limitation, or other circumstance that may affect safe participation. The participant will not participate while injured, ill, or medically restricted unless cleared by an appropriate medical professional. I understand that FUTSALTX does not provide medical insurance for participants. 4. Release and Waiver of Liability TO THE FULLEST EXTENT PERMITTED BY TEXAS LAW, I RELEASE AND AGREE NOT TO SUE FUTSALTX, ITS OWNERS, OPERATORS, AFFILIATED PROGRAMS, EVENT ORGANIZERS, FACILITY OWNERS, LANDLORDS, COACHES, OFFICIALS, EMPLOYEES, CONTRACTORS, VOLUNTEERS, SPONSORS, VENDORS, AND REPRESENTATIVES, COLLECTIVELY REFERRED TO AS THE “RELEASED PARTIES,” FOR CLAIMS ARISING FROM THE PARTICIPANT’S INVOLVEMENT IN FUTSALTX ACTIVITIES. THIS RELEASE INCLUDES CLAIMS ARISING FROM THE ORDINARY NEGLIGENCE OF A RELEASED PARTY, INCLUDING NEGLIGENT SUPERVISION, INSTRUCTION, FACILITY CONDITIONS, EQUIPMENT, EVENT OPERATION, OR FAILURE TO WARN. This release does not apply to gross negligence, reckless conduct, or intentional misconduct to the extent such claims cannot legally be waived. 5. Parent or Guardian Agreement for Minor Participants When signing for a minor participant, I confirm that: I am the participant’s parent or legal guardian or have legal authority to provide this consent. I give permission for the minor to participate. I understand and accept the risks on the minor’s behalf. I agree to the terms of this waiver individually and on behalf of the minor, to the fullest extent permitted by law. I accept responsibility for communicating all applicable rules and safety expectations to the minor. 6. Emergency Medical Authorization If the participant becomes injured or ill and I cannot be reached, I authorize FUTSALTX staff, coaches, officials, volunteers, or emergency personnel to: Provide reasonable first aid Contact emergency medical services Arrange transportation to a medical facility Authorize emergency evaluation or treatment when reasonably necessary I understand that FUTSALTX personnel are not required to provide medical treatment beyond their level of training. I accept financial responsibility for emergency transportation, treatment, hospital care, medication, or other medical expenses incurred on behalf of the participant. 7. Rules and Conduct I agree that the participant and accompanying family members will follow: FUTSALTX game and facility rules Instructions from coaches, referees, staff, and event officials Sportsmanship and spectator expectations Safety and equipment requirements Facility access and supervision policies FUTSALTX may remove or suspend any participant or spectator for unsafe behavior, fighting, abusive language, harassment, intentional misconduct, rule violations, or conduct that disrupts the event. Removal or suspension for misconduct may occur without refund. 8. Personal Property I understand that FUTSALTX is not responsible for lost, stolen, misplaced, or damaged personal property, including phones, bags, clothing, equipment, jewelry, or vehicles. 9. Photography and Media I understand that photographs or video may be taken during FUTSALTX activities. Unless I separately notify FUTSALTX in writing that I am opting out, I authorize FUTSALTX to use images or recordings of the participant for event coverage, social media, websites, promotional materials, and program documentation without compensation. The participant’s full legal name will not be intentionally published with an image without additional permission. 10. Indemnification To the fullest extent permitted by law, I agree to indemnify and hold the Released Parties harmless from claims, damages, liabilities, or expenses arising from: The participant’s actions or misconduct A violation of event or facility rules Damage caused by the participant Inaccurate or incomplete medical or registration information provided by me This provision does not require indemnification for conduct that cannot legally be released or indemnified. 11. Severability and Texas Law This agreement will be governed by the laws of the State of Texas. If any provision is found invalid or unenforceable, the remaining provisions will continue in effect to the fullest extent permitted by law. 12. Electronic Signature and Acknowledgment By signing or electronically accepting this agreement, I confirm that: I have read the entire waiver. I understand its terms. I understand that I am giving up certain legal rights. I am signing voluntarily. The information provided during registration is complete and accurate.
