Release of Liability and Assumption of Risk

Acknowledgment of Risk

I, the undersigned, desire that the participant, a minor of whom I am the parent and/or legal guardian, be permitted to participate in the Anderson High School Boys Soccer Program, including all activities, events, camps, conditioning sessions, training sessions, practices, matches, travel-related activities, and other program functions conducted by the Anderson High School Boys Soccer Program, its staff, coaches, trainers, Anderson High School, Anderson Community Schools, and their officers, employees, agents, contractors, and volunteers (collectively referred to herein as the “Program”).

I hereby authorize my child to fully participate in the Program, with the understanding that participation may include, but is not limited to, use of equipment, athletic facilities, fields, weight rooms, locker rooms, transportation, conditioning activities, competitions, and all other activities associated with the Program.

I understand that Anderson High School and Anderson Community Schools strongly encourage all participants to maintain adequate health and accident insurance coverage and to complete any required physical examinations prior to participation in the Program. I certify, based upon my own knowledge and consultation with a physician if necessary, that my child is physically able to participate in the Program.

I understand that participation in soccer and athletic activities involves inherent and significant risks that cannot be completely eliminated. These risks include, but are not limited to: sprains, strains, fractures, cuts, bruises, dislocations, concussions, dehydration, heat illness, communicable diseases (including COVID-19), cardiac events, emotional distress, permanent disability, paralysis, and death. I further understand that injuries or damages may result from participation in soccer-related activities, conditioning, training, contact with other participants, weather conditions, field conditions, equipment usage, travel, or the negligence of the Anderson High School Boys Soccer Program, its staff, coaches, trainers, Anderson High School, Anderson Community Schools, and their officers, employees, agents, contractors, or volunteers.

I voluntarily choose to allow my child to participate in the Program despite these known and unknown risks and assume full responsibility for any injury, illness, damage, or loss arising from participation.

Release of All Claims

In consideration of my child’s participation in the Program, I, on behalf of myself, my child, my heirs, representatives, executors, administrators, and assigns, hereby RELEASE, WAIVE, AND DISCHARGE the Anderson High School Boys Soccer Program, its staff, coaches, trainers, Anderson High School, Anderson Community Schools, and their officers, employees, agents, contractors, and volunteers from any and all claims, liabilities, demands, actions, causes of action, damages, costs, or expenses of any kind, including claims arising from alleged negligence, resulting from personal injury, illness, property damage, permanent disability, or death connected in any way with participation in the Program.

Indemnification

I further agree to DEFEND, INDEMNIFY, AND HOLD HARMLESS the Anderson High School Boys Soccer Program, its staff, coaches, trainers, Anderson High School, Anderson Community Schools, and their officers, employees, agents, contractors, and volunteers from and against any and all claims, demands, liabilities, damages, losses, costs, or expenses, including attorney fees and court costs, arising out of or related to my child’s participation in the Program.

Medical Authorization

I understand and agree that neither the Anderson High School Boys Soccer Program, Anderson High School, Anderson Community Schools, nor any of their officers, employees, agents, contractors, coaches, trainers, or volunteers assume responsibility for medical expenses incurred on behalf of my child.

In the event of an illness, injury, or medical emergency involving my child, I authorize Program staff, coaches, trainers, employees, agents, or volunteers to secure emergency medical treatment deemed necessary for my child. I understand that all costs associated with such treatment remain my sole responsibility.

I hereby release and hold harmless the Anderson High School Boys Soccer Program, its staff, coaches, trainers, Anderson High School, Anderson Community Schools, and their officers, employees, agents, contractors, and volunteers from any liability arising from decisions made in good faith regarding emergency medical care for my child.

Photo / Video / Audio Release

I hereby grant permission to the Anderson High School Boys Soccer Program, Anderson High School, Anderson Community Schools, and their representatives to photograph, videotape, livestream, or record my child during participation in the Program and to use such images, recordings, or likenesses for promotional, educational, athletic, school, website, social media, livestream, broadcast, or informational purposes without compensation.

I understand and agree that all photographs, videos, audio recordings, livestream footage, and related media become the property of the Anderson High School Boys Soccer Program and/or Anderson Community Schools.

Governing Law and Venue

This Release shall be governed by and construed in accordance with the laws of the State of Indiana. Any legal action arising out of this Release or participation in the Program shall be brought exclusively in the appropriate state or federal courts located within the State of Indiana.

If any provision of this Release is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

Agreement to the Terms of this Release

By selecting the acknowledgment checkbox and submitting this registration, I certify that I am at least 18 years of age and am the parent and/or legal guardian of the participant listed above. I acknowledge that I have carefully read and fully understand this Release of Liability and Assumption of Risk.

I understand that by electronically agreeing to this Release, I am waiving substantial legal rights on behalf of myself and the participant, including the right to recover damages for injuries, losses, or damages arising from participation in the Program, including those resulting from the ordinary negligence of the Anderson High School Boys Soccer Program, its staff, coaches, trainers, Anderson High School, Anderson Community Schools, and their officers, employees, agents, contractors, and volunteers.

By submitting this registration, I voluntarily agree to all terms and conditions contained in this Release and acknowledge that my electronic acceptance shall have the same force and effect as a handwritten signature.
