Temperature & Symptom Check-In Form

Please confirm that your Player does not have a Temperature before he or she leaves the house for this event.

Players CANNOT enter the Gifted Guard Clinic event facility with a temperature over 100.4 degrees or if they display any of the following symptoms: Cough, shortness of breath, lack of smell/taste or have unusual tiredness. Players also must put on hand sanitizer once they have been confirmed to enter the facility.

MAKE SURE TO PUT ON HAND SANITIZER BEFORE ENTERING FACILITY

For any players who do not pass the temperature and symptom check, they must Stay or go home and follow the self quarantine protocol detailed in the Clinic Covid-19 Safety Guidelines.
