Participation Waiver

I agree, by signing this form, to indemnify and hold harmless the Fairfield County Adult Basketball League, its officials, officers, employees and volunteers from and against any and all claims, actions, causes of action, loss, damage, injury, liability, cost or expense, including without limitation attorney’s fees arising out of, resulting from, or occasioned by participation in any activity associated with the Fairfield County Adult Basketball League.

Insurance: Fairfield County Adult Basketball League is not responsible and does not provide any insurance to any teams or individuals in the league.
