GMCO Youth Academy Sports Waiver

1. Code of Conduct

I understand that all participants are expected to behave in a respectful, sportsmanlike manner. I acknowledge that GMCO Youth Academy Sports reserves the right to remove a player from participation if they do not adhere to the team’s code of conduct or display inappropriate behavior.

1. Consent to Photography and Media

I understand that photographs and/or videos may be taken during practices, games, and events, and I give my permission for GMCO Youth Academy Sports to use such media for promotional, educational, or marketing purposes on their website, social media, or other platforms.

1. Governing Law

This waiver and release of liability shall be governed by and construed in accordance with the laws of the state of [Insert State Name].

1. Agreement

By signing this waiver, I confirm that I have read, understood, and voluntarily agree to the terms and conditions outlined above. I further agree that this waiver shall remain in effect for all basketball-related activities with GMCO Youth Academy Sports for the duration of the participant’s involvement with the team.
