2023 Home Team 941 Participation Waiver

PERMISSION FOR PUBLICITY
On occasion AMP United/ Home Team941, takes photographs or makes an audio or video recording of children and/or adults involved in activities. I consent to the use of any such audio or video record of the one named above to be used, distributed or displayed as agents of AMP United/ Home Team 941 see fit.

RELEASE OF LIABILITY
By signing this waiver, I grant permission for the child named above to participate in Home Team 941 activities. My child is physically and mentally able to participate in these activities. I acknowledge that there are certain risks involved in said activities and I have discussed them with my child.
I release AMP United, Inc., Home Team 941, its affiliates, volunteers, and employees of all responsibilities for any injuries to body or property which may occur to my child during the course of these activities. In the event of an emergency in which I or an alternate contact cannot be reached, I authorize the adult leaders to make the medical decisions for my child and to administer first aid, if necessary. I further agree to indemnify and hold harmless AMP United, Inc., Home Team 941 and its affiliates, volunteers and employees of any and all claims arising from my child's participation in activities or as a result of my child's injury or illness during such activities.
COVID
I understand and agree that my child will not be allowed to practice or participate in any activities if they are displaying any covid related symptoms. If my child tests positive for Covid-19, I agree to let management know ASAP so that we may attempt to isolate and avoid any further cases. By signing, I acknowledge I have discussed this with my child.

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF AMP UNITED, INC/HOME TEAM 941 USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM, YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT YO RECOVER FROM AMP UNITED, INC/HOME TEAM 941 IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM AND AMP UNITED, INC/HOME TEAM 941 HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN.

There are absolutely no refunds. We reserve the right to suspend and/ or expel any athlete from the program due to any type of negative behavior. Negative behavior examples could include:
disrespect to a coach, volunteer, referee, parent, or teammates/ drugs or alcohol use/ any behavior deemed as a danger to the athlete and/or the team.

I represent that I am the parent/legal guardian of the Athlete/Participant, who is under 18 years of age. If participant is 18 or over, I am signing this agreement for myself. I have read the Permission/Waiver and I am fully aware of its contents. I give permission for the child named to participate fully in the activities of AMP United, Inc/Home Team 941.
