By completing and submitting this form, and adding my electronic signature below, I hereby release my child to participate in practice and games for the Lady Warriors Youth Sports Volleyball league. I do not hold the school, the school district, or any other person affiliated with Watkins Memorial High School, Watkins Middle School, Watkins Intermediate School, Southwest Licking School District, or this league responsible for any injury or accident that may occur while my child participates in this league.
