Consent

I hereby give my consent for my child to participate in the MLJBL and agree that all coaches and other personnel associated with MLJBL shall be held harmless and not liable for any injury whatsoever sustained by my child in the activities of MLJBL. In the event of an emergency, I authorize MLJBL, its coaches and representatives to secure first aid and/or the service of any qualified physician or hospital for my child and agree to assume all financial obligations herewith.
I agree to protect, defend, indemnify and hold harmless the MLJBL (as well as its volunteers, contractors, officers, members, directors, employees, representatives, attorneys, and insurers) regarding any of the Claims, and shall be liable to pay attorneys' fees and costs incurred by the MLJBL or any of the foregoing persons mentioned in this paragraph in the event that I or any person on my behalf bring a Claim that has been otherwise waived by this agreement.
THIS AGREEMENT INCLUDES A WAIVER AND RELEASE OF LIABILITY. I HAVE READ THIS AGREEMENT. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I AM SIGNING THIS AGREEMENT ONLY AFTER A CAREFUL REVIEW OF ITS CONTENTS.
