Assumption of Risk

I understand that participation in pickleball involves inherent risks, including but not limited to: slips, trips, falls, collisions with other players or objects, being struck by a ball or paddle, muscle strains, sprains, fractures, heat exhaustion, dehydration, and other potential injuries. I acknowledge that these risks may result in serious injury, illness, or death. I voluntarily assume all risks associated with my participation in this tournament.

Physical Fitness

I affirm that I am physically fit to participate in the tournament and have no medical condition that would prevent safe participation.

Skill Level & Division Placement

I understand that I must register for the correct skill division based on my perceived ability, and if playing doubles, my team must enter the division of the higher-ranked player.

Release of Liability

In consideration of my participation, I hereby release, waive, and discharge Noorda-COM, the Orthopedic Surgery Interest Group, its students, organizers, volunteers, sponsors, and the City of Orem from any and all liability, claims, demands, actions, or causes of action arising from any loss, damage, injury, or death that may be sustained during or as a result of my participation in the tournament, whether caused by negligence or otherwise.

I agree to indemnify and hold harmless Noorda-COM, OSIG, its students, organizers, volunteers, sponsors, and the City of Orem from any loss, liability, damage, or costs they may incur due to my participation.

I grant permission for photographs and/or video recordings of me taken during the event to be used for promotional, marketing, or educational purposes, without compensation.
