Multimedia Permission

The undersigned hereby acknowledges that participation in this camp and related activities involves an inherent risk of physical injury, and the undersigned, on behalf of the registrant, hereby assumes all such risk and does hereby release and forever discharge the camp and all employees and agents thereof from any and all liability of whatever kind of nature, arising from and by reason of any and all known and unknown injuries, damage to property, and the consequences thereof, resulting from the registrant’s participation in or involvement with this camp, including and failure of equipment or defect in the premises.

Since the athletes attending the camp are under 18 years of age, it is necessary that the American Gold Sports Alliance has the permission of parents/legal guardians to administer treatment in the event of an accident or sudden illness. Specific guidelines and precautions have been taken in regard to the COVID-19 pandemic in order to assure the safety of our campers and coaches. I hereby authorize any medical treatment which may be advised or recommended for my camper while at the American Gold Sports Alliance Softball Camp.

I give my consent for the American Gold Sports Alliance (AGSA) to photograph or video my child taken in a sports camp setting. I understand that the photographs, digital images, or video segments taken during the camp are used for player development, player recognition and player motivational purposes for all of the camp participants, but also may be used in print or electronic media and that the photographs may be displayed on websites owned or sponsored by the AGSA. I give AGSA permission to publish, exhibit, and distribute these materials. Possible uses include educational, research, development, public service, or training activities, under the overarching vision and mission of the AGSA Program. I understand that AGSA owns the copyright to the multimedia material in which my child may appear. The AGSA will assure that all use of media conveys positive images of children and reflect the positive impact of sports on student-athletes.
