Participation Waiver

In consideration of my child’s participation in this league, I for myself, my child, my executors, and
administrators, and assignees do hereby release and discharge the AYVL, its board of directors, the coaches, the
referees, and its agents and employees from any and all claims for damages, causes of actions, whatsoever in any
manner arising out of my child’s participation in the AYVL. I state that I have full knowledge of the risk involved
on the participation in the AYVL, and my child is physically able and sufficiently trained to participate in this
league. I understand that participation in the AYVL is a privilege and subject to board of director approval.
