Waiver and Release of Liability

I, the legal parent/guardian, hereby agree to and grant permission for my child to participate in volleyball activities and performance training located at Beautiful Savior Lutheran 6995 W. 120th Ave, Broomfield, CO. I understand that sports/athletics is inherently vigorous and involves numerous physical demands and may involve contact, and therefore may cause injury. I furthermore certify and understand that my child is physically fit, in good physical health, and able to participate in volleyball training and sport performance training and gameplay. I understand that all attempts will be made to contact me in case of injury/illness. However, in the event I cannot be reached, I hereby authorize the staff of Ballistic Volleyball Academy and/or Beautiful Savior Lutheran to secure any and all medical treatment which may be deemed necessary to care for my child. I further authorize any attending physician to render any and all medical care which may be deemed necessary. I do hereby release and forever discharge and agree to indemnify Ballistic Volleyball Academy, their directors, coaches, staff and volunteers, Beautiful Savior, the owners of any facilities and equipment used, sponsors, directors, officers, representatives, agents, employees, and medical personnel of each from liability for any personal injury or illness, damage, or loss incurred while participating in the Ballistic Volleyball Academy program. I understand that I will be financially responsible for any and all damages to the facility, property, and equipment that are determined to be my child's fault.
I give my permission for BVA representatives to take pictures for promotional purposes only.
