Sports Liability Waiver Form

In consideration of being allowed to participate in any way in Brewer Youth Football sports program, games, voluntary workouts, events, and activities, I, (“Participant”), acknowledge and agree that:

1. I hereby acknowledge and confirm that my participation in the sports events and activities carries with it inherent and serious risks, including but not limited to property damage, bone fractures, slight and serious physical injuries, body sores, or death. I further agree that my participation in the sports events and activities is voluntary, and I assume all risks and responsibilities arising from my participation.

2. I understand and consent to be bound by the rules and policies explained by Brewer Youth Football in its Code of Conduct. If the Brewer Youth Football obtains competent proof of my violation of its Code of Conduct, whether negligently or intentionally, it reserves the right to expel and disqualify me from participating in its sports events and activities.

3. I discharge Brewer Youth Football, their officials, and employees, other participants, and authorized agents (“Releasees”), concerning any injury, illness, disability, death, and loss to person or property arising from my participation, whether arising from the negligence of the Releasees or others.

4. I promise not to sue and indemnify the Releasees from any liabilities arising out of my participation, whether arising from the negligence of the Releasees or others.

I have read this Sports Waiver, fully understand its terms, and acknowledge that by signing it, I have given up substantial rights, doing so freely and voluntarily.
