Player, Parent, Coach Waiver 2025-2026

Guidelines for players, coaches, and parents:
I will encourage good sportsmanship at all times by setting a positive example.
I will support all participants, including teammates, coaches, opponents, referees, and spectators.
I will keep winning and losing in perspective, be gracious, and applaud good effort.
I will strive to learn the rules of the sport and support the officials as they do their best to call a fair game.
I will demonstrate a positive attitude towards youth sports by controlling myself and keeping the best interest of the youth in mind as I encourage fair play, building teamwork, and learning.
If I identify a problem, I will calmly seek solutions at a proper time and location, refraining from negative confrontations. I will play hard for the fun of the game, encourage others, and respect all those around me.
WAIVER: I acknowledge that my child and/or I participate at our own risk. The undersigned is aware that there are certain inherent risks involved in participating in the Youth Basketball Program including injury. These inherent risks can be minimized by the exercise of good personal judgment and prudent action by the undersigned. Cache FUMC and all involved (including but not limited to school personnel and volunteers) are not responsible for any damage, injury, or death that may arise whether foreseeable or unforeseeable.
I hereby give my permission for any and all medical attention necessary to be administered to my child named in this registration form in the event of an accident, injury, sickness, etc. under the direction of the team coach or assistant coach until such time as I may be contacted. I also hereby assume the responsibility for payment of any such treatment.
The undersigned further grants permission for sponsors to use any photos, video tape, or any other record of this event for any purpose. Minors accepted only with parent/ guardian signature.
To the best of our ability, we will try to provide a safe environment for this league.
