Waiver

By initializing below, parent/guardian verifies that the registration information is correct and gives permission for the child to participate in the Turpin Cardinal Crazies' Youth activities.

In the event that a medical emergency occurs and I am not on the premises or cannot be contacted, I give permission to secure medical attention. Also, I do hereby release the Turpin Cardinal Crazies and all staff associated with Turpin Cardinal Crazies of all liabilities due to an injury or illness during practices or games.
