Description of Risks:
I, the undersigned participant or parent/guardian of the participant if the participant is a minor, acknowledge that participation in the elite youth baseball league organized by [Your League's Name] involves inherent risks, including but not limited to, the risk of:
Assumption of Risks:
I understand and accept that participation in baseball activities carries certain inherent risks of injury, illness, or death. I voluntarily assume all such risks and agree to abide by all rules, regulations, and instructions provided by [Your League's Name] and its representatives.
Waiver of Liability:

In the event of an emergency, I authorize league officials, coaches, or other authorized personnel to obtain medical treatment for me, including transportation to a medical facility, if deemed necessary. I understand that I am solely responsible for any medical expenses incurred as a result of such treatment.
Photography and Likeness:
I consent to the use of my name, likeness, and/or voice in photographs, videos, or other media recordings captured during league activities for promotional or informational purposes related to the league, without compensation or further approval.

Release and Waiver:
I, the undersigned, acknowledge and agree to the following terms and conditions:
I understand that baseball is a physically demanding sport and involves inherent risks of injury, including but not limited to:
Sprains, strains, and fractures
Concussions and other head injuries
Cuts, bruises, and lacerations
Heat-related illnesses
Lightning strikes
I have read and understood the league's rules and regulations, including safety protocols and emergency response procedures.
I acknowledge that my child or I (if over 18) voluntarily assume all risks associated with participation in the league, including the risk of serious injury or death.
I hereby release, waive, and hold harmless the league, its organizers, coaches, volunteers, and any other parties associated with the league from any and all claims, demands, or liability for any injuries, damages, or losses sustained while participating in league activities, regardless of whether such injuries, damages, or losses are caused by the negligence of the league or any other party.
I understand that the league reserves the right to remove any player from participation for disciplinary reasons or if the player's conduct or behavior poses a safety risk to themselves or others.
I grant permission for the league to use my child's or my (if over 18) name, photograph, and likeness for promotional or educational purposes.
Medical Authorization:
I authorize the league and its designated medical personnel to provide emergency medical treatment to my child or I (if over 18) in the event of an injury or illness during league activities.
I have provided the league with my child's or my (if over 18) complete medical history, including allergies, medications, and any relevant medical conditions.
COVID-19 Assumption of Risk:
I acknowledge the risk of contracting COVID-19 while participating in league activities.
I agree to follow all league protocols and guidelines to minimize the risk of exposure, including but not limited to:
Wearing a mask when required
Maintaining social distancing
Washing hands frequently
Staying home if experiencing any symptoms of COVID-19
I understand that if I or my child exhibits any symptoms of COVID-19, we will be required to stay home and follow CDC guidelines.

If any provision of this release and waiver form is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
