Indemnity Form

I acknowledge that I have familiarised myself with the nature and extent of the sport of dodgeball and the risks and possible dangers to which I may be exposed as a result of my participation in the sport. I voluntarily assume the risks and dangers to which I may be exposed as a result of my participation in dodgeball.
By signing this indemnity I agree not to hold Dodgeball SA, Nicholas Oebell or any Dodgeball SA employee or 3rd party liable for any injury, loss, damage, costs and/or expenses which I may sustain or incur as a result of my participation.
