Dream Team Basketball League Waiver

DREAM TEAM BASKETBALL LEAGUE (DTBL)

PLAYER WAIVER & RELEASE OF LIABILITY**

I acknowledge that participation in the Dream Team Basketball League (DTBL) is voluntary and involves inherent risks, including but not limited to physical contact, falls, collisions, overexertion, and the risk of serious injury.

By participating in DTBL activities, games, practices, or events, I knowingly and voluntarily assume all risks of injury, illness, or loss that may arise directly or indirectly from participation. I agree to accept full personal financial responsibility for any injury, loss, or damage I may sustain, including any medical treatment authorized by DTBL staff, volunteers, or representatives.

I hereby release, waive, discharge, and hold harmless Dream Team Basketball League (DTBL), its organizers, coaches, staff, volunteers, sponsors, and facility owners from any and all claims, demands, or causes of action arising out of or related to my participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I further agree to indemnify and defend DTBL against any claims brought on my behalf as a result of my participation.

Dispute Resolution

Any dispute or claim arising from this agreement shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association (AAA).

Photo & Video Release

I grant DTBL and its representatives the right to photograph or record me during league activities and to use such images or recordings for lawful purposes, including publicity, promotion, advertising, and web or social media content, without compensation.

I certify that I am 18 years of age or older, have read this waiver fully, understand its terms, and sign it voluntarily.
