The following includes BOTH the CBBR Photo Release and the Player Release for Chaos VBC, for Woks UP.

THE CANNON BEACH BUNNY RESCUE PHOTO RELEASE FORM

I hereby grant the CANNON BEACH BUNNY RESCUE permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of the CANNON BEACH BUNNY RESCUE and will not be returned.

I hereby irrevocably authorize the CANNON BEACH BUNNY RESCUE to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge CANNON BEACH BUNNY RESCUE from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW. I ACCEPT:

Portland Chaos VBC Club Waiver and Release of Liability 2026

I acknowledge that volleyball or any sporting event can cause potential death, serious injury, or property damage. With full understanding of the potential risks, I hereby assume the risks of participating. I hereby take the following action for myself, my executors, administrators, heirs, next of kin, successors, and assigns:
a) I waive, release, and discharge from any and all claims or liabilities for death or personal injury or damages of any kind, except that which is the result of gross negligence and/or wanton misconduct of persons or entities listed below, which arise out of or relate to my traveling to and from or my participation in any volleyball event, The following persons or entities: Portland Chaos VBC, Sand Woks Up Courts, Columbia Empire Volleyball Association, Dalzell Community Gym, Valley View Church, tournament directors, sponsors, and the officers, directors, employees, representatives, and agents of any of the above;
b) I agree to not sue any of the persons or entities listed above for any of the claims or liabilities that I have waived, released, or discharged herein; and
c) I indemnify and hold harmless the persons or entities mentioned above from any claims made or liabilities assessed against them as a result of my actions.
