1. Assumption of Risk
   I, [Participant's Full Name], understand that participating in the Umstead Volleyball League involves risks, including but not limited to falls, collisions, injuries from the ball, contact with other players, weather conditions, and other unforeseen hazards. I voluntarily assume all risks associated with playing in this league.

2. Release of Liability
   In consideration of being allowed to participate in the Umstead Volleyball League, I, for myself, my heirs, executors, and assigns, waive, release, and discharge the league organizers, Erin Connolly Coaching LLC, sponsors, referees, volunteers, property owners, and any associated entities or individuals from any and all liability, claims, demands, or causes of action related to injury, illness, disability, death, or property damage that may occur before, during, or after participation in the league.

3. Medical Treatment Authorization
   I understand that the Umstead Volleyball League does not provide medical insurance or coverage for injuries. In the event of an emergency, I authorize league organizers, referees, or volunteers to seek medical treatment for me if necessary. I assume all costs related to any necessary medical care.

4. Code of Conduct
   I agree to abide by the league’s rules and conduct policies, including respecting referees, fellow players, and the venue. I understand that failure to comply may result in suspension or removal from the league without a refund.

5. Media Release
   I grant permission to the Umstead Volleyball League to use photographs, videos, or recordings of me for promotional purposes on social media, websites, and other materials.

6. Governing Law & Severability
   This waiver shall be governed by the laws of the state of North Carolina. If any part of this agreement is deemed invalid, the remaining provisions shall remain in effect.

7. Parental/Guardian Consent (If Participant is Under 18)
   As a parent or legal guardian of the minor participant named below, I consent to their participation in the Umstead Volleyball League and agree to the terms outlined in this waiver.
