Liability Waiver and Code of Conduct for EVERYDAY ATHLETES SOCCER LEAGUE (Canada)

Assumption of Risk and Release of Liability:

I, the undersigned participant, wish to participate in the indoor soccer league organized by Everyday Athletes Soccer League ("the League"). I acknowledge that soccer involves physical exertion, the risk of bodily injury, and potential contact with other participants and equipment. I understand and accept that there are inherent risks associated with participating in physical activities, including soccer.

In consideration of being permitted to participate in the League, I hereby agree as follows:

I acknowledge and understand that participation in the League involves certain risks and dangers, including but not limited to the risk of injury, bodily harm, or death.

I voluntarily assume all risks associated with my participation in the League, including but not limited to those arising from the actions or negligence of other participants, the condition of the premises, and the use of equipment.

I release, waive, discharge, and covenant not to sue the League, its organizers, employees, agents, coaches, referees, and any other related personnel for any claims, demands, liabilities, actions, or causes of action, whether arising out of negligence or any other cause, that I may have for any injuries, damages, or losses sustained by me as a result of my participation in the League.

I understand that this waiver extends to any and all claims I may have against the parties released herein, including claims related to ordinary negligence.

Emergency Medical Treatment:

I authorize the League's staff and representatives to obtain emergency medical treatment for me in the event that I am unable to make medical decisions for myself. I understand that I am responsible for any medical expenses incurred.

Photography and Publicity:

I consent to the use of any photographs, videos, or other media taken during League activities for promotional, educational, or other purposes by the League.

Governing Law:

This waiver shall be governed by and interpreted in accordance with the laws of the province of Ontario, Canada.

Agreement to Terms:

I have read this liability waiver and fully understand its terms. I acknowledge that I am voluntarily giving up substantial legal rights by signing this waiver, and I do so freely and without any inducement or duress. I agree that this waiver is intended to be as broad and inclusive as permitted by the laws of the province in which the League operates.

CODE OF CONDUCT FOR PARTICIPANTS
Everyday Athletes Soccer League aim to provide all participants with a safe, competitive and enjoyable atmosphere for all participants.
To ensure this, all participants must agree to abide by the following code of conduct before, during and after all
games.

Participants are expected to treat everyone with courtesy and respect within the context of their sport, regardless of gender, place or origin, colour or ethnicity, religion, political belief, economic status or ability. This requirement prohibits any form of harassment or discrimination.

Participants must abide by the rules and regulations of their sport.

All participants are expected to refrain from any action that may endanger themselves, other participants, officials, spectators and staff members or cause damage to the equipment or facilities being used.

Dangerous play, whether deliberate or accidental will not be tolerated.

Verbal or physical abuse of officials or staff will not be tolerated.

Attempting to play while under the influence of alcohol or recreational drugs is not acceptable.

In entering a competition, teams and participants are committing to fulfill all necessary fixtures unless there are exceptional personal circumstances.

Any participant or team failing to abide by this code of conduct may face immediate or future suspension or expulsion from the game, league or program. No refunds will be issued in these circumstances.
