That in consideration of the above child being allowed to participate in the Falls City Recreation Inc. Program, the undersigned hereby acknowledges that he/she is a parent or guardian of the registrant above named. He/She acknowledges and agrees that Falls City Recreation, Inc. does not assume or accept any responsibility for any injuries or damages by reason of the registrant’s participation in the Falls City Recreation Inc. Program. The undersigned further acknowledges and agrees that he/she is fully aware of the dangers involved in the participation in the Falls City Recreation Inc. Program and hereby assumes the risk attendant thereto, and hereby releases Falls City Recreation, Inc., its referees, coaches, directors, and staff, Falls City Public Schools and the City of Falls City, Nebraska from any and all liability for any damages, to the registrant or his/her property and agrees to hold harmless authorized personnel of Falls City Recreation, Inc. for such damages.
