Consent sheet and health information release form

Customers should be aware that hiking is an activity with a degree of risk.

Please note that while we will take every reasonable precaution to prevent injury and loss of personal property, we can’t guarantee your safety.

You must release or indemnify Foris and our staff against any liability we incur to anyone else, resulting from your failure to follow our instructions or the failure of any person under the age of 18 accompanying you to follow our instructions.

Foris also reserves the right to cancel or amend any service at any time for any reason and assumes no liability for any loss, costs or consequent results.

This also serves as a record of who is on the trip.

All customers must sign their name and full address to acknowledge:

1. the said dangers and waive any liability of Foris and staff in respect of loss or damage to property, disruptions to travel plans or mental injury excluded to the extent permitted by the law. This exclusion is subject to any rights or remedies you may have under the Consumer Guarantees Act 1993.
2. their responsibility to tell us before the trip of any medical condition or physical disability which requires special treatment or medication. This includes allergies such as bee or wasp allergies.
3. that staff reserve the right to refuse to take anyone who appears to be under the influence of drugs or alcohol or whom they consider to be physically incapable of undertaking the trip.
4. where participants are under the age of 18, a parent or legal guardian must sign.
