ASSUMPTION OF RISK/WAIVER FORM- GEORGETOWN COLLEGE INTRAMURALS

I recognize that participation in activities sponsored and organized by the staff and student staff at Georgetown College carries certain risks including that of serious injury, which could result in paralysis and/or death. Further, I understand that Georgetown College does not provide insurance to cover medical or hospital expenses resultant from injury due to participation in the Georgetown College Recreation Program. I further understand that Georgetown College does not pay for ambulance or other transportation costs for me to obtain medical treatment in the event I should be injured.

Acknowledging the above, I freely and voluntarily choose to participate in these recreational activities understanding and appreciating the risks inherent in these activities and my participation therein. I waive any claims against Georgetown College, its agents, or employees should injury occur.
