Acknowledgment of Risk and Liability Waiver
As the parent or legal guardian of the participant listed above, I understand that participation in the Greater Southcoast Summer League (hereafter “the League”) involves inherent risks associated with athletic activity, including but not limited to:

Physical contact and collisions

Risk of sprains, fractures, concussions, or other injuries

The potential exposure to environmental conditions and illnesses, including COVID-19

I acknowledge that the League staff, coaches, volunteers, and venue hosts will take all reasonable precautions to ensure a safe playing environment. Nevertheless, I understand and agree that:

I voluntarily assume all risks associated with my child’s participation in the League.

I release and hold harmless the League organizers, including but not limited to Greater Southcoast Hoop Academy, facility staff, game officials, and volunteers, from any and all liability for personal injury, property damage, or loss sustained in connection with participation.

I understand that I am responsible for my child’s transportation, behavior, and medical needs during participation in the League.

I give permission for League staff to seek emergency medical care for my child if necessary, and I accept full financial responsibility for any medical treatment rendered.

Code of Conduct Acknowledgment
I further agree to support and encourage my child to:

Exhibit good sportsmanship at all times

Respect coaches, referees, and fellow participants

Follow all League rules and safety guidelines

Failure to adhere to League expectations may result in suspension or removal from participation without refund.
