Commercial Launch After Party

EMPLOYEE WAIVER AND PHOTOGRAPHY CONSENT AND RELEASE FORM

I understand that ING Business Shared Services B.V. Branch Office (hereafter referred to as “IBSS”) will be conducting a Commercial Launch After Party (the “Event”) on 21 May 2018 at The Island located at 9th Ave cor 36th Street, Uptown Bonifacio, Taguig. In this connection, I am voluntarily attending and participating in the Event, and warrant that I am fully aware of the nature of the activities in the Event and declare that I am physically fit to participate in the same.

In this regard, I hereby waive any action, sum of money, damages, claims and demands whatsoever, against IBSS and/or its successors-in-interest, stockholders, officers, directors, agents or employees, including organizers and participants of the Event, which in law or in equity I ever had, now have or which I, my successors and assigns hereafter may have by reason of any matter, cause or thing whatsoever in relation to my voluntary participation in the Event, the intention hereof being completely and absolutely to waive and release IBSS, as well as its successors-in-interest, stockholders, officers, directors, agents or employees, including organizers and participants of the Event, from all liabilities arising wholly, partially or directly from my voluntary participation in the Event.

I further acknowledge, understand, and consent to the following:

(1) In the course of conducting and documenting the Event, IBSS may collect my personal data (the “Personal Data”), including but not limited to my name and image (whether in photographic, video, or other form), and sensitive personal information.

(2) Such Personal Data may be used and processed by IBSS and its affiliates (collectively referred to as “ING”) for the publication of print or digital media, whether internal and external to ING, including but not limited to promotional, recruitment, and external market-related programs of ING, print publications, websites, e-marketing, posters, banners, advertising, film, and social media. ING may also make such Personal Data available to third parties who provide products or services to ING, to use or process my Personal Data in relation to the purposes described above.
(3) The Personal Data collected during the Event shall be retained in ING’s records and may be used or processed as described above during the period of my employment with ING and for a period of five (5) years from the end of my employment relationship with ING. Unless retention is required for a longer period for a reasonable cause, the Personal Data will be disposed upon the lapse of the 5-year period, in accordance with applicable law, regulations, and ING’s Global Data Protection and related Policies.
(4) I am entitled to certain rights in relation to any Personal Data collected from me during the Event, including the right to access, correct, object to the processing, as well as the right to lodge a complaint with the National Privacy Commission in case of a violation of my rights as a data subject.
(5) ING’s collection, use, processing, and disclosure of such Personal Data shall at all times be governed by applicable law and regulations and ING’s Global Data Protection and related Policies.
(6) I may consult the IBSS Data Privacy Officer at IBSS.DPO@asia.ing.com for any concerns regarding my Personal Data.

I finally declare that I have read and fully understood this document entitled Employee Waiver and Photography Consent and Release Form and have affixed my signature hereunder voluntarily and freely with full and complete knowledge of the import and intent of this document and of my rights under the law.
