📝 Joondalup Football Netball Club – Player Waiver and Acknowledgement
Applies to: Social Mixed Netball Competition
Effective: Per Season | Minimum Age: 16 Years

By registering to participate in the Joondalup Football Netball Club (JFNC) Social Mixed Netball Competition, I confirm that I have read, understood, and agree to the following:

⚠️ Assumption of Risk
I acknowledge that netball is a physical sport that carries a risk of personal injury, including but not limited to sprains, fractures, collisions, or other contact-related injuries.

I accept full responsibility for any injury, loss, or damage sustained by me as a result of my participation in the competition.

🛑 Player Fitness and Conduct
I confirm that I am in good physical health and medically fit to play netball.

I agree not to participate if I am injured, unwell, or under the influence of alcohol or drugs.

I understand that I may be removed from participation if I breach this condition.

🛡️ Liability and Insurance
I acknowledge that the competition is covered by public liability insurance only, and that personal injury insurance is not provided.

I release and indemnify Joondalup Football Netball Club, its volunteers, staff, and organisers from all liability for injury, loss, or damage arising from my participation, except where required by law.

🏥 Venue and First Aid
I understand that the competition is held at facilities hired from Edith Cowan University, and first aid is managed by venue staff, not JFNC.

I agree to follow any venue rules or instructions issued by ECU staff or JFNC representatives.

✅ Acknowledgement
By submitting this registration, I declare that:

I am aged 16 or over. If under 18, I have permission from a parent or guardian to participate.

I have read and understand this waiver.

I agree to be bound by this waiver for the duration of the season.

Note: This waiver is separate from the Player Code of Conduct, which all players must also agree to prior to participating.
