Softball Camp Disclaimer

As the parent or legal guardian of the registered participant, I hereby give my permission for my child/the child in my care to participate in the Lady Wolves Softball Camp. I understand that participation in sports involves inherent risks, including but not limited to physical injury, illness, and in rare cases, serious injury or death.

In consideration for allowing my child to participate in this camp, I hereby waive, release, and hold harmless Jamie Casarez, Jesse Casarez, all other coaches, student volunteers, staff, and organizers of the Lady Wolves Softball Camp, as well as any affiliated organizations or facilities, from any and all liability for injuries, accidents, or other claims that may arise from participation in camp activities.

I understand and agree that this release includes any claims based on the negligence, actions, or inactions of the coaches, volunteers, or camp personnel. I affirm that my child is physically capable of participating in this camp and that I will notify the camp staff of any health conditions or special needs that may affect their participation.

I also authorize camp personnel to seek emergency medical care for my child in the event I cannot be reached.

By signing below, I acknowledge that I have read and understand this waiver and agree to its terms.
