Lakes Region Leagues Waiver and Release

By signing this waiver. I acknowledge and agree to the following:

1. Age Verification:
I affirm that I am 18 years of age or older. I understand that Lakes Region Leagues is stritly for adult participants only. By registering and participating, I certify that I meet the minimum age requirement.

2. Voluntary Participation and Assumption of Risk:
I acknowledge that participation in recreational sports and activities involves inherent risks of physical injury, illness (including COVID-19), property damage, and even death. I voluntarily assume all such risks, whether known or unknown, even if arising from the negligence of the organizers.

3. Health and Pregnancy Disclosure:
I certify that I am physically able to participate in league activities and have not been advised otherwise by a qualified medical professional.

If I am pregnant, suspect I may be pregnant, or have any health condition, I understand that participation in physical activities may carry additional risks.

I take full responsibility for my participation and release Lakes Region Leagues from any liability related to injuries or complications due to pregnancy or other medical conditions.

4. Release of Liability:
In consideration of being permitted to participate, I hereby release, waive, discharge, and covenant not to sue Lakes Region Leagues, its officers, agents, employees, volunteers, sponsors, and other participants from any and all liability for injury, disability, death, loss, or damage to person(s) or property arising from my participation, whether caused by negligence or otherwise.

5. Medical Authorization:
I authorize Lakes Region Leagues representatives to secure emergency medical care on my behalf if deemed necessary. I accept full responsibility for any medical expenses incurred.

6. Personal Responsibility:
I agree to follow all leagues rules, respect other participants, and maintain sportsmanship. I understand that failure to comply with rules or conduct standards may result in removal from the league without refund.

7. Photo and Media Release:
I grant Lakes Region Leagues permission to use photographs, video, and other media of me participating for promotional purposes without compensation.

IMPORTANT
By signing below, I acknowledge that I have read, fully understand, and voluntarily agree to this waiver and release of liability. Additionally, by typing my name below, I acknowledge and agree that this serves as my legal e-signature, and I consent to the terms and conditions of this waiver.
