Participant Waiver

I, the undersigned player, acknowledge, agree, attest that:

1. I am 18 years of age or older, or I am the legal guardian of the participant.

2. Voluntarily and of my own free will, I elect to participate as a member of the bowling team and league at Lam's Lanes, Newburg ND.

3. I understand that there are certain risks and hazards involved in participating in bowling that may result in injury or death to me or other players including, but not limited to those hazards associated with bowling, lanes, equipment, and other participants.

4. I am aware that this activity may be hazardous and am voluntarily participating in this activity with knowledge of the risks involved, both expected and unexpected, and hereby agree to accept any and all risks of loss or personal injury.

Further, I, the undersigned player, agree that in consideration for the right to play as a member of a team and as part of the bowling league, and in consideration for permission to play on the lanes arranged for the team or league:

A. I voluntarily elect to accept and assume all risks of injury incurred or suffered by me
(1) while practicing or playing as a member of the team so designated,
(2) while serving in a non-playing capacity as a team member during practice or play by other teams or by other players on my team, and
(3) while on or upon the premises of any and all of the fields arranged for by my team or league for practice or play.

B. I release, discharge and agree not to sue or bring any legal action against the team, team players and Lam's Lanes, Newburg ND, or its agents, servants, associations, employees, or any person or entity connected with the team for any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by me from whatever cause including, but not limited to, the negligence, breach of contract or wrongful conduct of the parties hereby released.

I, the undersigned player, affirm that I am voluntarily participating in the Garage Bar & Bowl bowling league. In return for the opportunity to participate, I agree not to sue and hereby release and agree to hold harmless Lam's Lanes, Newburg ND, its employees, its agents, and any volunteers working with the University for and from liability and responsibility for any loss or injury connected with my participation in the
Bowling League, including reasonable attorney’s fees and cost whatsoever, including those for personal injury, death or property damage, which may arise from or in connection with participating in this softball league.

Further, I hereby give Lam's Lanes, Newburg ND, permission to use any photographs taken at any of the events surrounding Bowling League and I am not entitled to compensation of any kind for the use of such photographs.

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability, hold harmless agreement, and assumption of risk agreement and that it is a legally binding contract between Lam's Lanes, Newburg ND, and me. I further understand that this release is binding on my heirs, assigns, next of kin, executors, representatives, or anyone making a claim on my behalf. I further agree to abide by the rules and regulations of the Bowling League. I sign this waiver of my own free will and by doing so certify that I have read this agreement.
