I am the parent or legal guardian of the child identified above (“Participant”). I give permission for my child to participate in running, walking, games, drills, fitness activities, practices, and other activities organized by Life Run Club LLC(“Life Run Club”).
I understand that Life Run Club is a youth recreational running and fitness program and that participation involves physical activity.
I understand that running, exercise, outdoor recreation, and children’s athletic activities involve inherent risks that cannot be completely eliminated even when reasonable precautions are taken.
These risks may include, but are not limited to:
I understand that injuries can occur despite reasonable supervision and safety precautions.
I knowingly and voluntarily permit my child to participate with an understanding of these inherent risks.
I believe my child is physically capable of participating safely in Life Run Club activities.
I agree to inform Life Run Club of any medical condition, allergy, physical limitation, medication, or other circumstance that Life Run Club reasonably needs to know to help my child participate safely.
I understand that Life Run Club and its coaches and volunteers do not provide medical evaluations or medical advice.
If my child’s health or physical abilities change in a way that could affect safe participation, I agree to notify Life Run Club.
I understand that Life Run Club will provide reasonable adult supervision during scheduled program activities.
I understand that coaches and volunteers may be supervising multiple children simultaneously and that continuous one-on-one supervision of my child is not provided or guaranteed.
I understand that activities take place in a public park that may remain open to other members of the public.
My child is expected to remain with the designated group and follow reasonable instructions from Life Run Club coaches and volunteers.
I understand that a parent or guardian is not required to remain at the park during the scheduled program unless Life Run Club communicates otherwise.
Life Run Club’s supervision of my child begins when my child is checked in or otherwise accepted by a Life Run Club coach or designated representative at the scheduled activity.
I agree that my child will not be dropped off before the designated check-in time unless prior arrangements have been made.
Life Run Club’s supervision ends when my child is released to me or another adult I have authorized to pick up my child.
I agree to pick up my child promptly at the scheduled end of the program.
I understand that Life Run Club may request identification before releasing a child to an individual whom its staff or volunteers do not recognize.
I have discussed with my child the importance of following instructions from Life Run Club coaches and volunteers.
Participants are expected to:
I understand that Life Run Club may require a parent or guardian to pick up a child or may restrict future participation if the child’s behavior creates a significant safety concern.
If my child becomes injured or ill during a Life Run Club activity, I authorize Life Run Club coaches, volunteers, or representatives to provide reasonable basic first aid within their abilities.
If I or the emergency contact listed above cannot be reached promptly, I authorize Life Run Club to contact 911 or other emergency medical services when reasonably believed necessary.
I authorize emergency medical personnel to evaluate and provide emergency treatment to my child when medically appropriate.
I understand that Life Run Club does not guarantee the presence of a medical professional at practices or activities.
I understand that I am responsible for medical expenses incurred for my child, subject to applicable law and insurance coverage.
I understand that Life Run Club activities may be modified, postponed, or canceled because of severe weather, unsafe field or park conditions, excessive heat, thunderstorms, poor air quality, or other circumstances that Life Run Club determines may affect participant safety.
I understand that children should avoid bringing unnecessary valuables to Life Run Club activities.
Life Run Club is not responsible for lost, misplaced, stolen, or damaged personal belongings except to the extent otherwise required by law.
I have carefully read this Youth Participation Agreement.
I understand the nature of Life Run Club activities and the inherent risks associated with running, exercise, outdoor recreation, and youth athletic activities.
I voluntarily give permission for my child to participate.
I understand that this agreement is intended to document my informed consent, acknowledgment of inherent risks, emergency medical authorization, and the responsibilities of participants and parents/guardians. Nothing in this agreement is intended to waive rights that cannot lawfully be waived under New York law.
I certify that I am the child’s parent or legal guardian and have authority to provide this consent.
