Waiver and Liability Release

WAIVER AND LIABILITY RELEASE FORM
Lighthouse Community Church (LCC) is committed to hosting recreational programs/activities in a safe manner and holds safety of participants in high regard. LCC strives to reduce risk and insist that all participants follow safety rules and instructions to reduce inherent risks that come along with recreation activities. As a participant or parent/guardian of a minor participating in any recreational programs I understand that not all hazards and dangers can be foreseen. Injuries due to accident, slipping, falling, poor skill level, conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate supervision, instruction, officiating or other circumstances inherent to indoor or outdoor recreation/activities exist. In this regard it is impossible for LCC to guarantee absolute safety.

I recognize and acknowledge that there are certain risks of physical injury to participants and I voluntarily assume full risk of any and all injuries, damages, or loss, regardless of severity that my minor child/ward or I may sustain as a result of said participation. I further agree to to waive and relinquish all claims I or my minor child/ward may have (or occur to me or my child/ward) as a result of participating in these programs/activities against LCC including its officials, agents, volunteers, employees or partners.
