Youth Football Waiver and Release of Liability

1. Release of Liability

I, the undersigned, understand that participation in youth football involves risk of injury, including but not limited to scrapes, bruises, sprains, broken bones, concussion, or other serious injuries. I assume full responsibility for any injuries, losses, or damages that may occur as a result of my child’s participation in this program.

I hereby waive, release, and discharge Louisiana Aces, its coaches, volunteers, sponsors, and affiliates from any and all liability, claims, or causes of action for any injuries or damages arising out of my child’s participation.

1. Medical Authorization

In the event of a medical emergency, I authorize the coaches or representatives of Louisiana Aces to secure emergency medical treatment for my child, including transportation to a hospital or medical facility, and I accept full responsibility for any related medical expenses.

1. Photo & Video Release

I give permission for photos and videos of my child participating in team activities to be used for promotional purposes on social media, websites, or printed materials.

1. Acknowledgment and Consent

By signing below, I acknowledge that I have read and understand this waiver and agree to its terms. I certify that my child is in good physical condition and able to participate in youth football activities.
