Soccer

PARTICIPANT LIABILITY WAIVER, RELEASE, AND MEDICAL AUTHORIZATION

1. ASSUMPTION OF RISK
   I understand that participating in soccer involves inherent risks of injury, including but not limited to sprains, fractures, concussions, and severe physical trauma. I voluntarily choose to participate (or allow my child to participate) in this tournament and assume all risks associated with training, games, and travel to and from the event.
2. LIABILITY RELEASE & INDEMNITY
   I hereby release, waive, and discharge this Tournament, its organizers, directors, referees, coaches, volunteers, and the owners of the fields/facilities from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, occurring during this event. I agree to indemnify and hold harmless the released parties from any claims brought by me or on behalf of the participant.
3. MEDICAL EMERGENCY AUTHORIZATION
   In the event that I cannot be reached in an emergency, I hereby grant permission to the tournament staff, coaches, and medical personnel to authorize and administer necessary medical treatment, X-rays, anesthesia, or surgery to the participant.
4. PHOTO AND VIDEO RELEASE
   I grant the Tournament the irrevocable right to take and use photographs, video recordings, or digital media of the participant during the event for promotional, marketing, or website purposes without compensation.
5. SAFETY POLICY COMPLIANCE
   I agree to follow all tournament safety protocols, including immediate removal from play if a concussion or head injury is suspected. I certify that the participant is physically fit to compete in competitive soccer matches.

ACKNOWLEDGEMENT
By checking the agreement box and providing my digital signature below, I certify that I am at least 18 years old, have read this document completely, understand its terms, and agree to be bound by them as a participant or as the legal parent/guardian of the minor participant.
