Tournament Release Form

Recognizing the possibility of injury associated with soccer and in consideration for the Wappinger United Soccer Club, I hereby release, discharge, and/or otherwise indemnify Wappinger United Soccer Club against any claim by or on behalf of the registrant as a result of the registrant’s participation in the tournament. Further, I affirm that my child is physically capable of participating in the tournament.
