Mid-Michigan 3 on 3 Basketball Waiver & Release of Liability

By registering for and participating in Mid-Michigan 3 on 3 Basketball events, I acknowledge and agree to the following:

1. Assumption of Risk
I understand that participation in basketball activities involves inherent risks, including but not limited to physical injury, illness, or other unforeseen circumstances. I voluntarily assume all risks associated with participation.

2. Release of Liability
I agree to release, waive, and hold harmless Mid-Michigan 3 on 3 Basketball, its organizers, staff, volunteers, affiliates, and facility partners from any and all liability, claims, demands, or causes of action arising out of participation in this event.

3. Medical Responsibility
I certify that I (or my child) am physically able to participate. I understand that no medical insurance is provided and I am responsible for any medical expenses that may arise.

In the event of an emergency, I authorize event staff to obtain necessary medical treatment.

4. Conduct & Safety
I agree to follow all rules, instructions, and decisions made by event staff. I understand that unsafe behavior, unsportsmanlike conduct, or misconduct may result in removal from the event without refund.

5. Media Release
I grant permission for photos and videos taken during the event to be used for promotional and marketing purposes by Mid-Michigan 3 on 3 Basketball.

6. Minor Participants & Team Registration
If registering a team or multiple participants under the age of 18, I confirm that I am a parent, legal guardian, coach, or authorized representative of the participant(s), and I have obtained permission from each participant’s parent or legal guardian to register and accept this waiver on their behalf.

I understand that I am responsible for ensuring that all participants on my team have parental/guardian consent to participate.

Agreement
By completing registration, I confirm that I have read, understood, and agree to this waiver and release of liability.
