Northeast Metro Grassroots Soccer Waiver

I am the parent/guardian, of the child being registered to play in the Northeast Grassroots Soccer League. As lawful consideration for my minor child being permitted to participate in the Northeast Metro Grassroots Soccer League or any other activity through the Northeast Metro Grassroots Soccer. I agree that neither my minor child nor I will make a claim against, sue, attach the property where games or practices are played, or prosecute those involved with the Northeast Metro Grassroots Soccer League for damages for death, personal injury or property damage which my minor child may sustain as a result of my child's participation in these sporting activities. This release is intended to discharge in advance anyone involved with the Northeast Metro Grassroots Soccer League, and the city parks where practices and games are played from and against any and all liability, including for negligent actions, arising out of or connected in any way with my minor child's participation in the sports league, program, camp, clinic or any other activity. I FURTHER UNDERSTAND THAT SPORTS INVOLVE PHYSICAL CONTACT BETWEEN PLAYERS, THAT SERIOUS ACCIDENTS OCCASIONALLY OCCUR DURING SUCH SPORTING ACTIVITIES, AND THAT PARTICIPANTS IN SUCH SPORTING ACTIVITIES OCCASIONALLY SUSTAIN SERIOUS PERSONAL INJURIES (INCLUDING DEATH) AND/OR PROPERTY DAMAGE, AS A CONSEQUENCE THEREOF. KNOWING THE RISKS OF PARTICIPATION, NEVERTHELESS, I HEREBY AGREE THAT MY MINOR CHILD AND I ASSUME THOSE RISKS AND RELEASE AND HOLD HARMLESS NORTHEAST METRO GRASSROOTS SOCCER AND THE CITY PARKS INVOLVED WHO (THROUGH NEGLIGENCE OR CARELESSNESS) MIGHT OTHERWISE BE LIABLE TO ME, MY MINOR CHILD (OR OUR HEIRS OR ASSIGNS) FOR DAMAGES. I attest that I am eighteen (18) years old or older and that my child is physically fit and has no known medical conditions which prohibit participation in this sport. My child and I agree to follow all laws, rules and guidelines regulating the conduct of the league, camp or clinic. I understand and agree that my child and I are responsible for the mechanical and/or operating condition of any and all sporting equipment provided by my child or by me for my child's use, and I agree that my child and I will continuously inspect and maintain all equipment used, even if we have obtained any of the equipment from Northeast Metro Grassroots Soccer. I ALSO UNDERSTAND THAT THE NORTHEAST METRO GRASSROOTS SOCCER LEAGUE IS RUN BY PARENTS, SO I AGREE TO HAVE A PARENT OR GUARDIAN PRESENT AT ALL GAMES, PRACTICES, AND ACTIVITIES HELD BY NORTHEAST METRO GRASSROOTS SOCCER – I AM RESPONSIBLE FOR MY CHILD. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY FOR MYSELF AND MY CHILD AND A CONTRACT BETWEEN MYSELF, MY CHILD AND NORTHEAST METRO GRASSROOTS SOCCER, AND I HAVE SIGNED IT OF MY OWN FREE WILL.

I acknowledge the contagious nature of Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Northeast Metro Grassroots Soccer (NMGS) has put in place preventative measures to reduce the spread of Coronavirus/COVID-19.
I further acknowledge that NMGS can not guarantee that I will not become infected with Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by Coronavirus/COVID-19 may result from my child or myself attending NMGS practices or games.
I acknowledge that I must comply with all set procedures to reduce the spread while attending NMGS practices and games.
I attest that:

My child and I are not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
My child and I will not attend any NMGS practices or games if we, or anyone else in our family, are exhibiting any symptoms of illness.
My child and I do not believe we have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
My child and I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.
My child and I will follow all CDC recommended guidelines to reduce the risk of exposure while participating in NMGS activities. I hereby release and agree to hold NMGS harmless from, and waive on behalf of myself, my child, and any other family members who may attend NMGS activities any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or child that may be caused by any act, or failure to act of NMGS, or that may otherwise arise in any way in connection with NMGS. I understand that this release discharges NMGS from any liability or claim that I, my child, or any family members may have against NMGS with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to NMGS.
