Phoenix Flames Player Waiver & Liability Release

I, the undersigned player or parent/guardian, hereby acknowledge that participation in Phoenix Flames basketball activities involves physical activity and risk of injury. I agree to assume full responsibility for any injuries, damages, or loss that may occur while participating. I release Phoenix Flames, Georgia Ortega, coaches, staff, and volunteers from any liability arising from participation. I understand that proper safety measures will be in place, but participation is at my own risk. I consent to emergency medical treatment if necessary. I also give permission for photos or videos taken during Phoenix Flames events to be used for team promotion and social media.
