By signing this Participant Waiver, you are assuming certain risks and waiving certain rights, including a right to sue. Please read it carefully before signing it. EVERYONE must sign this document before playing in Safe Harbor Easton's tournament - sponsored by Hormel Foods.
As a condition of using Paxinosa Elementary's facilities, I agree to
1) adhere to applicable rules, regulations and policies;
2) exercise reasonable care and good judgment in using the facilities; and
3) observe the prescribed rules and hours of use.
I acknowledge that activities taking place during a basketball tournament have inherent risk of injury, including, but not limited to, those related to contact with the court, equipment surrounding the court, contact with other individuals on the
court, contact with equipment on the court, including the hoops and the equipment used by those on the court.
I further assume all risks, including injury and property loss as a result of my participation in this event, and do release Safe Harbor Easton, Hormel Foods, and their agents/employees, on behalf of myself, my executors and assigns, from any claim or cause of action arising out of or related to my participation in the Safe Harbor Easton's tournament.
I acknowledge that the basketball tournament I am participating in is not affiliated with, organized by, or supervised by Safe Harbor Easton. By signing below, I agree that Safe Harbor Easton is solely a beneficiary of this event. Hormel Foods and Safe Harbor Easton are in no way liable or responsible for any injuries.

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.
