Acknowledgment of Participation
I, the undersigned parent/guardian, hereby give permission for my child to participate in the Fall 2025 Shenandoah 3-on-3 Basketball League. I understand that participation in any athletic activity involves inherent risks of injury. I agree to assume full responsibility for my child’s participation in this program.
League Format
The league consists of 6 weeks of regular season games followed by a Championship Tournament.
Each team will play two 12-minute back-to-back games per week during a 30-minute “game block.”
Teams will be placed into scheduled blocks (4:30, 5:00, 5:30, or 6:00 PM).
Games are played under 1s and 2s scoring rules with no referees; staff supervisors will be present.
Fouls will result in restarts or free throws per league rules.
Health & Safety
I certify that my child is physically fit to participate and that I have informed league organizers of any relevant health conditions.
I understand that Shenandoah Country Club staff will take reasonable precautions to promote safety, but injuries may still occur.
Payment & Refund Policy
I understand that my account will be charged $75 for my child’s participation in the league.
I understand that this payment is non-refundable after Week 1 of the season.
Liability Release
As a member of Shenandoah Country Club, I acknowledge that this program follows the same participation rules as all other club activities. I release Shenandoah Country Club, its staff, and representatives from liability in accordance with the club’s standard participation policies.
Apparel & Team Colors
Each player will receive a league t-shirt (size collected at registration).
Teams are expected to wear the same colors for all games.
Backup pennies will be provided if necessary.
Awards
Members of the 1st place team will each receive a $50 Mixed Grille gift card and medals.
Members of the 2nd place team will also receive medals.
Code of Conduct
I understand that players, parents, and spectators are expected to maintain good sportsmanship. Failure to do so may result in removal from the league without refund.

By signing this waiver, I confirm that I have read, understand, and agree to the above terms.
